The St. John-St. Paul Collaborative must adhere to the following requirements concerning both volunteers and staff. These safeguards have been set up by civil and Archdiocese of Boston authorities to protect our children and youth, as well as the elderly and disabled.

We are grateful to every volunteer who gives of their precious time to help us to do God’s work in and beyond our Collaborative. We thank you for your generous service to our Collaborative community, and for taking the time to complete these requirements so that we can work together for the safety of our most vulnerable parishioners.

CORI Form  (Annually)

It is mandated by Massachusetts state law that all Collaborative staff and volunteers submit a Criminal Offender Record Information (CORI) background check each year to ensure the safety of children, youth, elderly, and the disabled. Since it is necessary for you to show a photo identification (license), CORI forms have to be completed in person at the Parish Office or with a member of the Collaborative staff.

A completed CORI is required for all new volunteers before beginning service, and CORI checks are repeated annually for all volunteers, usually in the spring.

To complete the form, your government-issued ID must be checked by a Collaborative employee.  The staff person who coordinates your volunteer service may arrange a time for this, or you can come in to the parish office during regular business hours.  Alternatively, you may bring your ID and the form to a Notary Public who will verify your identification and sign and seal the form, after which it can be returned to the parish office.

The Archdiocese of Boston issues a new CORI form each year. The form for the current year can be accessed via this link: CORI Form FY2024
